I am getting this error message: "one or more files were not added because those files already exist on a list"

This is not possible as I just downloaded from itunes. In any case, the list icons are grayed out . I cannot find a pref file in the app library (I am on a mac osx)

It's likely that you're attempting to convert an audio file that has DRM protection (i.e. files purchased through iTunes), and hence cannot be converted. Also, ensure that you are running the latest version of Switch.
