EMPTY BAR in front of Coda mark?

I use D.S. al Coda to send Crescendo back for a third verse. Up to "Segno" it goes. It plays until it finds, "To Coda." Then it jumps to the Coda symbol (circle with cross in it). Movement of playback is good, but the score has an "empty" bar in front of the Coda. This empty bar never plays. If I put notes in it, they never sound.

Is the Coda supposed to have an empty bar preceding it?

Would this issue happen with all the score you create?

If you open one of the sample scores, and you add the DS al Coda, would the issue also happen?
